Common:

• Fever, chills

• Nausea, vomiting, anorexia, weight loss

• Skin changes: hyperpigmentation, erythema, rash, thickening, ulceration

• Hair loss (alopecia)

Serious / Severe:

• Pulmonary toxicity (interstitial pneumonitis, pulmonary fibrosis major dose-limiting effect, may be fatal)

• Severe hypersensitivity or anaphylaxis (especially in lymphoma patients)

• Bone marrow suppression (usually mild compared to other chemotherapy drugs)

• Pulmonary Monitoring: Perform baseline and periodic chest X-ray & pulmonary function tests. Risk of fibrosis increases with cumulative dose.

• Cumulative Dose Limit: Lifetime dose usually should not exceed 400 IU.

• Renal Function: Adjust dose in renal impairment.

• Elderly Patients: Higher susceptibility to pulmonary toxicity.

• Pregnancy & Breastfeeding: Contraindicated (teratogenic).

• Surgery/Anesthesia: Supplemental oxygen increases pulmonary complicationsuse with extreme caution.

• Drug Interactions: Avoid concomitant use with other pulmonary toxic agents (cisplatin, cyclophosphamide in high doses, oxygen).
