2. Product Description

Alkeran 50mg Injection contains Melphalan, an alkylating cytotoxic chemotherapy agent derived from nitrogen mustard.

It works by alkylating DNA bases, leading to DNA cross-linking, inhibition of DNA and RNA synthesis, and ultimately cell death.

It is primarily used in high-dose conditioning regimens before hematopoietic stem cell transplantation (HSCT) and in the treatment of multiple myeloma.

• Multiple Myeloma standard treatment, alone or in combination with prednisone or other agents.

• High-Dose Therapy for Stem Cell Transplantation used as conditioning regimen in autologous or allogeneic bone marrow transplant.

• Ovarian Cancer in advanced or relapsed cases (less common than oral form).

Common:

• Nausea, vomiting, diarrhea

• Oral ulcers, stomatitis

• Loss of appetite

• Fatigue, weakness

Serious / Severe:

• Bone marrow suppression severe neutropenia, thrombocytopenia, anemia

• Risk of serious infections due to immunosuppression

• Pulmonary toxicity (fibrosis, pneumonitis rare but life-threatening)

• Hepatotoxicity (veno-occlusive disease with high-dose therapy)

• Secondary malignancies (long-term use risk: leukemia, MDS)

• Hypersensitivity reactions (rare)

6. Precautions

• Monitoring: CBC, liver function, renal function must be checked regularly.

• Renal Impairment: Dose adjustments required; melphalan is renally excreted.

• Hepatic Impairment: Caution advised.

• Pregnancy & Breastfeeding: Contraindicated (highly teratogenic, mutagenic, carcinogenic). Reliable contraception required.

• Handling: Cytotoxicuse protective equipment during preparation and disposal.

• Drug Interactions: Avoid live vaccines; additive toxicity with other bone marrow-suppressive or hepatotoxic drugs.
