Arsenic trioxide is an antineoplastic agent used in the treatment of acute promyelocytic leukemia (APL). It induces apoptosis and promotes the differentiation of malignant promyelocytes. • Induction: 0.15 mg/kg IV once daily until bone marrow remission or for a maximum of 60 days

• Consolidation: 0.15 mg/kg IV once daily for 25 doses over 5 weeks (5 days per week), beginning 36 weeks after induction

• Infusion: Dilute with suitable IV fluid; infuse over 12 hours

• Adjustments: Interrupt or modify dosage for grade 3 toxicity, cardiac arrhythmia, or organ dysfunction

Common:

• Nausea, vomiting, diarrhea, fatigue, fever

• Myelosuppression: anemia, neutropenia, thrombocytopenia

• Differentiation syndrome (APL syndrome): fever, dyspnea, pulmonary infiltrates, hypotension, renal failure (requires immediate corticosteroid treatment)

• Electrolyte imbalances: hypokalemia, hypomagnesemia
